To clear up any concerns regarding vacation swaps there is a new form that will need to be used for all future vacation week swaps. The form will be available in Line Control and the CSM’s will have copies too.
The form is simple to fill out; your name, your employee number, and the week (swap) you currently hold – the next line is the person you are swapping with; their name, employee number and the week (swap) they currently hold. Be sure to have a CSM sign and date the form and leave it with them to process. Make a copy for your records !!
As a reminder; any weeks may be swapped that have not been proffered, bidded, or awarded through the Vacation Relief process. It is not necessary to post this form or get any other approval other than the signing CSM.
